How to Make an Appointment


Step One
Most Carle Therapy Services require a physician referral so you will need to talk to your physician about being referred to Carle Therapy Services.

Step Two
Once your physician has made the referral, the Carle Therapy Services staff will contact you in order to schedule an appointment that is convenient for you.

Step Three
If this is your first appointment, please review the Helpful Hints section for some handy tips and reminders.

Helpful Hints for your First Appointment

  • Please arrive 15 minutes before your appointment to allow yourself enough time to complete the necessary paperwork.

  • Please remember to bring your insurance card or proof of insurance to your appointment.

  • If your appointment is with wheelchair evaluation, driver assessment or urinary incontinence, you should receive a survey packet in the mail a week before your appointment.  Please bring the completed survey with you to your appointment.  If you have not received a packet, please call (217) 326-2400 for a wheelchair evaluation or driver assessment survey, or (217) 383-3400 for the urinary incontinence survey.

  • Wear comfortable clothing that allows your body to move freely.

  • If your treatment includes water therapy, please remember to bring your suit and other swimming devices – earplugs, swim cap, etc.

  • Bring your enthusiasm – a positive attitude plays a big part in the healing process.
